Medical Authorization

In the event of an emergency, I authorize AZ Glow Baseball League representatives to obtain medical treatment for the participant if a parent or guardian cannot be reached.

Waiver and Release of Liability

In consideration of being permitted to participate in AZ Glow Baseball League activities, I, on behalf of myself, my child, my heirs, assigns, personal representatives, and next of kin, hereby release, waive, discharge, and covenant not to sue AZ Glow Baseball League, its owners, officers, directors, employees, coaches, volunteers, officials, sponsors, advertisers, partners, host facilities, school districts, municipalities, property owners, and affiliated organizations from any and all liability, claims, demands, actions, causes of action, damages, costs, expenses, attorney fees, losses, or injuries arising out of or related to participation in league activities, including those resulting from ordinary negligence.

Assumption of Risk

I understand that participation in baseball and related activities involves inherent risks, including but not limited to: injury, illness, property damage, or death. Risks may arise from playing conditions, equipment, weather, collisions, contact, the actions of others, or other unforeseen circumstances. I voluntarily assume all known and unknown risks associated with participation, including those associated with nighttime and blacklight (UV) play.

League Policies

Photo & Video Release

I grant AZ Glow Baseball League permission to photograph, videotape, livestream, and use the participant's image, likeness, and voice for promotional, marketing, social media, and other league purposes without compensation.

Code of Conduct

I agree to conduct myself with respect and sportsmanship at all times. Violations may result in disciplinary action, suspension, or removal from the league without refund.

Concussion Policy

Any participant exhibiting signs of a concussion will be removed immediately. Return-to-play requires written medical clearance from a licensed healthcare provider.

Weather Policy

Games and practices may be postponed or canceled due to weather or field conditions. We will make reasonable efforts to reschedule when possible.

Refund Policy

All registration fees are non-refundable unless required by law. Refund requests prior to team formation may be considered on a case-by-case basis.

Safety & Equipment

Participants must use equipment safely and responsibly. The league reserves the right to inspect equipment and require removal of unsafe or illegal items.

Acknowledgement

I have carefully read this agreement and all policies listed above. I understand its contents and agree to abide by all league rules, policies, and decisions. I understand that I am giving up certain legal rights and sign this agreement voluntarily.
